I, the undersigned, hereby waive any and all future claims and do release the City of Pocatello, a municipal corporation of Idaho (hereinafter “the City”), and any and all officers, employees, volunteers, agents, insurers and any elected and/or appointed officials of the City from any and all civil liability for any and all forms of injury which may arise as a result of any fostering activities with the Pocatello Animal Services.
I understand that there are many known and unknown dangers and/or risks associated with my fostering in the above program, and I hereby release for myself, my heirs, executors, administrators and assigns,. I additionally waive, remise and forever discharge and release the City and any and all other officers, employees, volunteers agents, insurers, and any elected and/or appointed officials of the City from any and all claims, demand, right, cause of action, property damage, personal injury, costs, loss of service, expenses of any kind, and any compensation whatsoever, several or otherwise, past, present, or future, which can or may be asserted as a result of my presence and/or participation in fostering with Pocatello Regional Animal Services. I have read the foregoing document and understand that the terms of this release are contractually and legally binding and that no verbal statement to the contrary, by any person or entity, can void or alter the terms of this release.
I have read this application and waiver in its entirety and agree that all statements and agreements within this document made by me are truthful under penalty of perjury under the laws of the state of Idaho.
