Instructions

Prior to completing this form, you must read the Conditions of the Concessions Permit (PDF) document. Afterward, answer each question on the application thoroughly. Once you have completed the form and attached all applicable documents, your application will be reviewed by Parks & Recreation staff who will contact you.

Provide a detailed description of the concession to be used (i.e. stand, cart, trailer, table with canopy, etc.). Dimensions of any freestanding unit are required.

Provide a detailed listing of items/food/beverages to be sold or given away.

List any special requirements you have.

Provide a detailed description of your recycling plan. If you will not have any recyclable materials, enter N/A. If you will have recyclable items, you must have a plan for proper disposal of those items. Include a list of recyclable materials that will be generated and how those recyclable materials will be collected and processed.

Acknowledgment

Copies of the following documents may be required prior to issuance of permit:

Public Liability Insurance Policy (minimum of $1,000,000) naming the City of Pocatello as an additional insured
Southeast Idaho Public Health Permit or Exception (food and beverage concessions only)
Letter of Determination (nonprofit organizations)
